Abstract
A focus on the healthcare experience and improvement of the continuum of care is not a new one. Underinsured patients who have no access to clinicians, utilize hospital emergency departments (EDs) as primary care, are admitted into the hospital and are discharged with little long-term plan for care. This becomes a cycle. With data and literature supporting the need for improved care coordination, this paper will focus on a Community Benefit program that uses WellSpace Health (local Federally Qualified Health Center or FQHC) case managers to work with underinsured patients at high-risk for readmission. Building upon the success Sutter Medical Center, Sacramento program, T3 (Triage, Transport and Treatment), T3+ will endeavor to improve efficiency and outcomes and reduce hospital readmissions by approximately twenty-five percent among a 5-10 high-risk patient population within a six-month period. If successful, this model can be replicated and expanded.
As the Regional Community Benefit Manager for the Sutter Health Sacramento Sierra Region, the researcher has completed this study with knowledge of community benefit programs, the underinsured, healthcare and the Sutter Health system.
